Online back-to-school re-registration opens Monday, June 3. Our focus for early summer re-registration is the families of 7-12th grade athletes and 9-12th grade Marching Band participants - other families are encouraged to register in July or August. Parents can complete the required back-to-school forms for fall as well as required OHSAA forms for sports participation at the same time. An online forms Open House is planned for June 4.

Our online forms will create less paperwork, dramatically reduce redundancy within forms, pre-populate content, and include year-to-year/sibling-to-sibling carry over of key information.

Account Creation

Student signatures: Middle and High School students may use their district-assigned email address to sign into FinalForms to complete the student signature portion of the forms. 

Missing Parent Email Addresses: A parent within each household should have received a FinalForms invitation on May 31. Families who did not receive an invitation may not have an email address on file. If you encounter any problems, please email This email address is being protected from spambots. Fall athletes

Our Crestview Athletics Packet is now incorporated within FinalForms online forms. In addition to being able to submit all of the required OHSAA documents, student health information will carry over onto the printable OHSAA Physical questionnaire packet that families can take to the student's sports physical. In future years, families will need only to update the previous year's content to complete the requirements.

Students planning to attend the free Avita Sports Physical event on June 10 should complete their online forms and print the OHSAA PPE form prior to the event. 

Marching Band

Families of students participating in Marching Band Camp will want to complete back-to-school forms ahead of camp. Completing this process eliminates the separate Emergency Medical Form previously required for participation. 

Open House

An optional Open House for any family who would like assistance in completing the online forms is planned for Tuesday, June 4, from 5:30-7:00 PM at the Middle School Media Center. While we believe most families will find the online system easy to use, this event is offered to ensure everyone is comfortable with the process. If you feel confident in completing the forms on your own, please do!

Participants are encouraged to bring any information they may need to complete the student health questionnaire and list emergency contacts. Additional Open House events are planned for later this summer. Further support can be obtained by emailing Chad Lemon
