The role of the clinic is to provide the best possible care to your student and promote good health. Some of the tasks school nurses handle are responding to emergencies, educating students, and yearly screenings. The district’s nurses strive to make sure your student is getting the care they need while at the school, as well as communicating with parents and staff on student needs. As a member of the educational team, the school nurse enhances students' potential for attendance, class participation, and learning by identifying and addressing medical conditions that may impact the student's ability to achieve success.g. 

Guidelines for Medications

If medication needs to be taken during the school day, please follow these guidelines:

  • Have a completed, signed Medication Authorization Form, found on the "Medications" tab in FinalForms.

  • The parent/guardian must complete the bottom section and a healthcare provider must complete the medication order section.

  • A new Medication Authorization Form must be completed each school year or when there is a change in the medication or dose.

  • All medication must be in the original container in which it was dispensed by the prescribing physician, healthcare provider or licensed pharmacist.

    • The container must be labeled with the correct dose and instructions.

    • The label should match what is on the Medication Authorization Form.

    • Students taking medication at both school and home can request 2 separate labeled bottles from the pharmacy to divide the pills to have some at home and school.

  • Aside from over-the-counter medications pre-authorized in FinalForms, school personnel cannot give students over-the-counter medications unless prescribed by a healthcare provider and a Medication Authorization Form is completed.

  • Prescribed over-the-counter medications follow the same guidelines as stated above for prescribed medications.

  • Medications ordered three times a day or less, unless there is a time specified, may not need to be given at school. The medication should be given before school, after school and at bedtime.

  • All leftover medication must be picked up by the parent or guardian on the last day of school or it will be disposed of properly.

Immunizations

Ohio law requires students to have up-to-date immunizations prior to entering school, and again at grades 7 and 12, or families must provide a completed exemption form.

Families can upload a scan or photo of their student's immunization record by clicking on the "File Folder" icon in FinalForms.

Screenings

As required by the Ohio Department of Health, screenings will be conducted for students in the following grades:

  • Kindergarten: Vision & Hearing

  • First Grade: Vision & Hearing

  • Third Grade: Vision & Hearing

  • Fifth Grade: Vision & Hearing

  • Seventh Grade: Vision ONLY

  • Ninth Grade: Vision & Hearing

  • Eleventh Grade Vision & Hearing

Student Illness

As per the CDC and school policy, all students with a fever of 100 degrees or greater will be sent home. They may not return to school until they are fever free for 24 hours without the use of Tylenol, Motrin, etc. Students with active vomiting or gastrointestinal problems will be sent home and may not return to school until their problem has been resolved for 24 hours. Our policy is enforced to promote the well-being of all students and staff members. Please advise the school if your child has any contagious diseases, major illnesses, or any surgery throughout the school year. In many cases, a doctor’s note will be required for your child to return to school. If your child is excluded from gym/recess for a medical reason, a doctor’s note will be required to provide the exclusion. A doctor's note will also be needed before your child may resume activities.
