Crestview Local Schools is pleased to offer preschool to our district residents. This enriched learning environment is provided in an integrated setting for children between the ages of 3-5 by August 1.
Parents interested in enrolling their child in Crestview’s preschool can begin by completing the Request for Preschool Enrollment form (FinalForms will be completed later in the process). Returning preschool students should complete the Request for Preschool Enrollment form as well.
All enrollment requests should be completed prior to preschool screening.
In March, parents will be notified whether their child is considered eligible for the upcoming school year. Preference will be based on age. At this time we will schedule a screening appointment.
Community Preschool Screenings for the 2024-2025 school year will be held beginning in late March.
After the screening, if admitted, you will then complete the enrollment process through FinalForms.
Information required for registration:
Birth Certificate
Current Immunization Record
Photo ID of Parent/Guardian
Custody Papers (if applicable), and
Two Proofs of Residency in the Crestview School District (a complete list of acceptable proofs of residency can be viewed on the Registration page)
Proof of income (if applying for tuition assistance)
Once the required documents have been presented and confirmed enrollment, you and your child(ren) will be set up in Final Forms, our online enrollment system. You may complete these forms from home or in our office if you don’t have access to a computer at home. Please call 419-895-1700, extension 17002, to inquire about the registration process.
